New Member Work Ticket Procedure
Click : On the BLANK NEW MEMBER WORK TICKET ICON in left margin of desktop
Enter Date, CA Number, Last Name, First Name, Phone Number, Type of Issue and Member Issue.
If it is determined that the work will be done at the Member's home, check the Home Visit checkbox and enter the address and email.
Click: File PRINT
Click : File S AVE A S
Click : On
NEW MEMBER WORK TICKET
IN THE FILE NAME AREA, START TYPING THE LAST NAME, FIRST OF THE CLIENT. (There is no need to erase the blue area—just type)
Click : S AVE
Click : X in upper right corner to close
Click : O n the Blank Liability Release Form I con in left margin of the desktop and complete the Liability Release Form using instructions on a separate page.
